Below you can download the Application Form template to apply as a Partnership Broker within the UA_EU_CLUSTER5.0 project.

Complete all sections carefully and sign where requested.

Attach all required documents (CV, self-certification, portfolio, references, motivational statement, etc.) as specified in the Selection Criteria.

Send the completed Application Form and all annexes by email to: ua.eu.cluster5.0@gmail.com
Please remember that the deadline for submitting your application is 30 December 2025, 23:59 CET. Applications received after this deadline will not be accepted.
